New patients at A Healing Hand Physical Therapy need to complete several forms before their initial appointment.
We ask that you download and fill these forms out ahead of time. Please remember to bring them all with you to your first appointment.
In order to plan a therapeutic treatment that is safe and effective we ask that you please be as thorough as possible when filling out your Medical History form.
Also please bring with you any reports or additional medical information related to your condition that your physician may have given you.
